This form requires applicants to upload their admission requirements, and once submitted, will no longer allow edits. It is recommended to complete all admission requirements first before filling-out this form.

1. Academic credentials, e.g. TOR, Form 137 and 138, ALS or PEPT Certificates.

2. NSO or PSA Birth Certificate.

3. Certificate of Good Moral Character.

While online submission of application requirements is accepted for now, all admitted applicants are required to submit the hard copy of admission requirements along with 4 2x2 picture and Income Tax Return of either of the student’s parent (for High School only) before the end of the semester to:
